Overview

Pathfinder Recovery is seeking an experienced professional to join our team in a flexible, part-time remote role supporting individuals on their journey to recovery.

In Short

• Coordinate care for clients in CT, MA, VT, or NH.
• Assess needs and develop actionable service plans.
• Connect clients to critical local resources.
• Maintain consistent contact via phone, video, and secure messaging.
• Facilitate referrals to external providers.
• Advocate for client needs including employment support.
• Use AI-enhanced documentation tools to track progress.
• Occasionally meet clients in person.

Requirements

• Prior experience in SUD case management or client care coordination.
• Familiarity with recovery systems and best practices.
• Personal recovery experience is highly valued.
• Strong communication and relationship-building skills.
• Comfort with technology and telehealth platforms.
• In-depth knowledge of recovery resources in CT, MA, VT, and NH.
• A client-centered, empowering approach to care.
