Overview

The Nursing Community Housing Outreach Coordinator is responsible for coordinating healthcare activities and ensuring quality services for individuals in supported housing, focusing on those with mental health and substance use disorders.

In Short

  • Coordinate healthcare for individuals in scattered site supported housing.
  • Conduct health assessments and screenings.
  • Provide health education to program participants.
  • Collaborate with medical providers.
  • Document all required healthcare activities.
  • Respond to medical emergencies and coordinate assistance.
  • Review medical records for compliance.
  • Serve as a liaison between participants and healthcare providers.
  • Maintain regulatory compliance and facilitate educational programs.
  • Travel to clients' homes throughout Westchester County.

Requirements

  • Current RN or LPN license in New York State.
  • Two years of nursing experience in mental health or related fields.
  • Valid CPR/BLS/AED certification.
  • Strong communication and organizational skills.
  • Experience with adults in mental health.
  • Ability to travel and work flexibly.
  • High level of patience, empathy, and compassion.
  • Excellent observation and problem-solving skills.
  • Understanding of healthcare state standards.
  • Strong computer skills.
