The Care Management Associate supports comprehensive coordination of medical services, promoting effective utilization of healthcare services and ensuring quality effectiveness.

• Coordinates Care Team intake and screenings.

Requirements

• Experience with Foster Care and juvenile justice systems.
• 2-4 years’ experience as a medical or office assistant.
• Proficiency in MS Office Suite applications.
• Effective communication and organizational skills.
• Familiarity with medical terminology and care management concepts.
• Strong customer service skills.
• Ability to work in a multi-disciplinary team.
• High School Diploma or G.E.D required.
