Overview

As a Care Navigator at Vivo Care, you will build relationships with patients, utilizing motivational communication techniques to drive positive health behavior change and improve patient outcomes through remote monitoring.

Requirements

  • Unrestricted RN/LPN/LVN license in a compact state and New York.
  • Ability to work in the EST time zone.
  • 1-3 years of clinical experience in patient management.
  • Experience with virtual visits and telephonic care management.
  • Spanish speaking is a plus.
  • Fast learner with the ability to work independently.
  • Experience with Medicare patients is desirable.
  • Interest in health behavior change and motivational interviewing.
  • Health Coach certification is a plus.
  • Willingness to contribute to process development in RPM care.
