Overview

As a Specialty Care Navigator for Substance Use Disorder (SUD) at Spring Health, you will play a pivotal role in supporting individuals with substance use disorders. This full-time remote position allows you to leverage your licensure and advanced skills to manage client care and coordinate support services.

In Short

  • Full-time remote position reporting to the Clinical Manager of the SUD Program.
  • Conduct outreach and follow-ups with individuals seeking SUD treatment.
  • Engage clients using advanced clinical techniques to assess needs.
  • Coordinate appointments and manage complex cases for clients.
  • Provide psycho-education and motivational interviewing.
  • Utilize knowledge of ASAM levels of care and SUD treatments.
  • Maintain accurate records within an EHR system.
  • Collaborate with interdisciplinary teams for integrated care.
  • Participate in staff meetings and ongoing training.
  • Adhere to confidentiality and ethical standards.

Requirements

  • Mental Health Licensure required: LCSW, LMFT, LPC with 2+ years of SUD clinical care experience.
  • Proficiency in motivational interviewing and ASAM criteria.
  • Strong communication skills and ability to work with diverse populations.
  • Comfortable using various computer platforms.
  • Ability to manage multiple tasks efficiently.
