Overview

As a Health Coach, Enrollment, you will be responsible for high-volume outbound outreach to eligible program participants and enrolling patients into a high-touch health program supporting their medication adherence and lifestyle changes.

In Short

  • Communicate with patient prospects via phone call, text, and e-mail to introduce them to Scene services for easy enrollment.
  • Leverage best-practice listening skills to identify patient needs.
  • Guide patients through the enrollment process with motivational support.
  • Manage patient information within Salesforce.
  • Overcome patient objections and technical barriers.
  • Assist patients with technical setup and app usage.
  • Connect patients to health resources.
  • Maintain detailed records of patient interactions.
  • Build performance goals with management.
  • Provide feedback on patient challenges for process improvement.

Requirements

  • 3+ years of experience in health education or patient engagement.
  • Background in health coaching or community health work.
  • Strong interpersonal skills for empathetic conversations.
  • Excellent verbal and written communication skills.
  • Experience with phone, SMS, and video outreach.
  • Ability to explain health concepts simply.
  • Proficiency with database systems and mobile applications.
