Our technology-driven care platform enables us to engage patients early, conduct individualized risk assessments for poor pregnancy outcomes, and deliver coordinated, personalized virtual care throughout pregnancy, NICU stays, and the first postpartum year. We measure ourselves by reductions in preterm births, NICU admissions, c-sections and maternal mortality; we improve outcomes and reduce healthcare spend.

In Short

• Provide direct patient care and clinical oversight for pregnant people and newborns.
• Accountable for improving clinical outcomes for empaneled patients.
• Attend daily clinical huddles with a collaborative clinical team.
• Review complex patient cases and develop care plans.
• Monitor adverse events and identify areas for improvement.
• Lead development of evidence-based medical protocols.
• Participate in on-call schedules including overnight and weekends.

Requirements

• A minimum of 4 years obstetrics experience as an APP.
• Passion for optimizing evidence-based care and perinatal outcomes.
• Experience using data to drive patient engagement and outcomes.
• Experience leading successful teams with outstanding collaboration.
• Exceptional organization and attention to detail.
• Facility using multiple tech platforms.
• Eager to thrive in a fast-paced, metric-driven environment.
