
The National Association of Chronic Disease Directors (NACDD) is a nonprofit, member-based organization dedicated to enhancing public health through improved state leadership and expertise in chronic disease prevention and management. Comprising 59 State and Territorial Health Department Chronic Disease Directors and their teams, NACDD focuses on primary and secondary prevention efforts while addressing the root causes of chronic conditions. The association also connects over 7,000 chronic disease professionals across various sectors, including state, tribal, and territorial health departments, nonprofits, academia, and the private sector. NACDD advocates for health promotion, provides education, and offers technical assistance to bolster chronic disease prevention knowledge and capacity nationwide.
The Program Manager will oversee CDC-funded projects focused on chronic disease prevention and health promotion, ensuring effective planning, implementation, and evaluation.
