Staff Physician - Remote

Overview

We are seeking a mission-driven team player passionate about empowering PACE participants to live the independent lives they deserve. In this role, you'll serve as a vital member of the Interdisciplinary Team (IDT), collaborating to provide holistic care that meets participants' unique needs.

In Short

• Serve as an integral member of the Interdisciplinary Team (IDT).
• Deliver direct primary medical care to PACE participants.
• Initiate, evaluate, and close orders for medications and treatments.
• Provide care in various settings including homes and skilled nursing facilities.
• Coordinate comprehensive care plans with IDT members.
• Assist participants during hospital stays and nursing home transitions.
• Educate participants and caregivers on medical care.
• Initiate referrals to specialist providers.
• Perform selected in-office procedures.
• Maintain thorough documentation in the EMR.

Requirements

• Strong commitment to providing compassionate care to seniors.
• At least 1 year of experience caring for frail and elderly populations.
• Excellent interpersonal and communication skills.
• Ability to lead independently and thrive in a team environment.
• Graduate of an accredited School of Medicine or Osteopathy.
• Completion of an ACGME accredited residency program.
• Active, unrestricted MD/DO license in California.
• Board Certification in Internal Medicine or Family Practice.
• Current CPR certification.
• Valid California driver’s license and personal transportation.
