Licensed Practical Nurse (LPN) - Member Health Assessor - Remote

Overview

We are seeking a Licensed Practical Nurse (LPN) who will work remotely and in the field to support our Medicaid SMI (Severe Mental Illness) Population in South Carolina.

In Short

• Work with Medicaid members to complete care plans and assessments.
• Partner with Behavioral Health Case Managers.
• Excellent computer skills and attention to detail required.
• Fast-paced position with productivity expectations.
• Up to 60% travel required in the field.
• Home office with high-speed internet required.
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM EST.
• 1-3 years of experience in relevant settings required.
• Active and unrestricted LPN license required.

Requirements

• Completion of an accredited LPN program or relevant degree.
• 1-3 years of experience in case management or behavioral health.
• Valid driver's license and good driving record.
• Ability to work with members with complex clinical needs.
• Experience with Health Risk Appraisals.
• Ability to meet daily production standards.
• Experience with web-based documentation systems.
• Preferred: 3-5 years of experience in relevant settings.
• Preferred: Relevant certifications (e.g., LCSW, CCM).
