Overview

The Home Health Care Coordinator at Molina Healthcare Services will support members with high need potential by coordinating integrated care across various health services, including behavioral health and long-term care.

In Short

  • Support Washington State Plan as a Registered Nurse.
  • Conduct comprehensive assessments and develop case management plans.
  • Perform ongoing monitoring and outreach to members.
  • Facilitate interdisciplinary care team meetings.
  • Utilize motivational interviewing techniques.
  • Manage a caseload of members with complex medical conditions.
  • Conduct home visits as required.
  • Promote integration of services for continuity of care.
  • Local travel (25-40%) required.
  • Must have valid RN license in Washington State.

Requirements

  • Graduate from an Accredited School of Nursing.
  • 1-3 years of experience in case management or related fields.
  • Active, unrestricted RN license in good standing.
  • Valid driver's license and reliable transportation.
  • Bachelor's Degree in Nursing preferred.
  • 3-5 years of experience preferred.
  • Certified Case Manager (CCM) preferred.
