The Home Health Care Coordinator at Molina Healthcare Services will support members with high need potential by coordinating integrated care across behavioral health and long-term care, ensuring quality and cost-effective outcomes.

Requirements

• Completion of an accredited LVN or LPN Program or a relevant Bachelor's/Master's Degree.
• 1-3 years of experience in case management or related fields.
• Active and unrestricted license if required.
• Valid driver’s license and reliable transportation.
• Preferred 3-5 years of experience in similar settings.
• Preferred licenses include LCSW, CCM, LPC, etc.
