  • Manage patient care plans and documentation.
  • Coordinate with interdisciplinary teams for patient care.
  • Complete medication reconciliation and maintain accurate profiles.
  • Design and implement project plans for operational efficiency.
  • Communicate effectively with management and staff.
  • Report any observed or suspected abuse as mandated.
  • Support the mission and goals of the company.
  • Perform additional tasks as assigned.

Requirements

  • Current state license as a Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Language Pathologist.
  • Detail-oriented with strong organizational skills.
  • Excellent written and verbal communication skills.
  • Adaptable and supportive team player.
  • Ability to pass a background check.
