Overview

We are seeking Nurse Care Managers for our Care and Case Management team, who are passionate about providing holistic care and guiding members through their healthcare journey.

In Short

• Deliver coordinated, patient-centered virtual Care Management via telephone or video.
• Create impactful care plans with members and the care team.
• Help members navigate complex medical conditions and the healthcare system.
• Partner with local providers for coordinated care.
• Provide compassionate, longitudinal follow-up care.
• Assist during acute healthcare episodes.
• Coordinate resources to address clinical and social problems.

Requirements

• Bachelor of Science in Nursing (BSN).
• Current CCM Certification or eligibility for exam in August 2025.
• 5+ years of experience in nursing.
• 2+ years of experience in care management.
• Must reside in a compact NLC state.
• Active Compact RN license in good standing.
• Willingness to maintain licensure in multiple states.
• Comfortable discussing various medical conditions.
• Spanish speaking is desirable.
• Experience working remotely and using medical record systems.
