Overview

The Connected Care Assistant plays a crucial role in providing world-class service to members with complex health conditions by coordinating care and support services in collaboration with a clinical team.

In Short

  • Coordinate patient-care services for members.
  • Engage with at-risk members through calls and chats.
  • Use autodialer for targeted outreach.
  • Manage care plan task queues.
  • Recommend benefits and initiate related cases.
  • Complete administrative tasks and communications.
  • Attend training and comply with healthcare regulations.
  • Maintain patient confidentiality and follow HIPAA guidelines.
  • Work flexible hours, primarily 11-8 EST or 8-5 PST.
  • Other duties as assigned.

Requirements

  • Prior experience in case management or healthcare preferred.
  • Proficiency in motivational interviewing techniques.
  • Strong telephone and communication skills.
  • Ability to multitask and adapt in a changing environment.
  • Highly organized with effective time management skills.
  • Proficient in computer technology and health IT systems.
  • Knowledge of community healthcare resources preferred.
  • Positive attitude towards peers and patients.
  • Acknowledge patient rights and maintain confidentiality.
  • Commitment to scheduled hours.
