Therapist (LCSW, LMFT, LMHC or LCAC) - Remote

Overview

The Therapist (LCSW, LMFT, LMHC or LCAC) provides information, assessment, crisis intervention and other mental health services to assigned patients, developing individualized treatment plans.

In Short

• Completes diagnostic summaries and communicates assessment findings to the treatment team.
• Determines if a person needs crisis intervention or long-term care.
• Counsels patients and families regarding emotional, social and financial consequences of illness.
• Provides mental health counseling including crisis intervention and short-term counseling.
• Maintains patient records with treatment notes and continuing care plans.
• Participates in continuing education programs for professional growth.
• Develops professional referral relationships and resources for patient support.
• Works in a hybrid environment, required to work one day a week in the health center.

Requirements

• Requires LCSW or equivalent license and a Masters in a related field.
• Four to five years of therapy experience required.
• Knowledge of child/adult protective services and legal issues.
• Knowledge of mental health treatment and community resources.
• Electronic charting experience preferred.
• Strong customer service and communication skills.
• Ability to work with diverse populations.
• Reliable transportation may be required.
