Nurse Practitioner – Palliative Care Program - Remote

Overview

As a Nurse Practitioner (NP) in our Palliative Care Program, you will provide expert care to patients facing acute and chronic conditions, working closely with physicians and interdisciplinary care teams.

In Short

• Manage day-to-day operations of the Palliative Care Program.
• Coordinate patient services with a multidisciplinary team.
• Assist in the orientation and development of clinical staff.
• Provide comprehensive palliative care through health assessments and care plans.
• Work collaboratively to create individualized care plans.
• Discuss end-of-life issues and provide comfort to patients and families.
• Make referrals or consult with specialists as needed.
• Utilize community resources for patient care.

Requirements

• Active California RN License and Certified Nurse Practitioner status.
• Strong experience in palliative care or hospice.
• Excellent skills in assessment, diagnosis, and treatment.
• Strong interpersonal communication skills.
• Highly organized and self-directed.
