Halcyon Home is seeking a dedicated and compassionate Part Time Triage Registered Nurse (RN) to provide remote triage and clinical support to hospice patients and families. We offer competitive pay and wonderful team support.

In Short

• Provide remote triage and clinical support to hospice patients and families.
• Conduct thorough triage assessments of patient symptoms and needs via telephone.
• Offer symptom management guidance and education to patients and caregivers.
• Coordinate with on-call hospice team members for timely care.
• Ensure accurate documentation in the patient care system.
• Provide emotional support to patients and families.
• Evaluate urgency of care needs and escalate cases when necessary.
• Communicate effectively with interdisciplinary teams.
• Requires a current TX or Compact State RN license.
• Minimum of 2 years of hospice or acute care experience.

Requirements

• Must have current TX or Compact State RN license.
• Minimum of 2 years of hospice, palliative care, or acute care experience.
• Prior triage experience preferred.
• Strong communication, problem-solving, and critical thinking skills.
• Proficiency in electronic medical record (EMR) systems.
• Ability to work independently with empathy and professionalism.
