In this pivotal role, you will play a crucial part in our Patient Management Program, focusing on individuals with specialized and complex health conditions.

• Assess and evaluate the conditions of our members through telephonic interactions.
• Develop and implement comprehensive care plans.
• Monitor patient progress through regular assessments.
• Utilize advanced telecommunication systems to provide medication education.
• Work independently while adhering to established protocols and guidelines.
• Maintain a set schedule with inbound and outbound tasks.
• Occasional travel to Humana's offices for training or meetings may be required.
• Work hours include late nights and holiday rotations.

Requirements

• Bilingual English/Spanish.
• Bachelor of Science in Nursing.
• 3 - 5 years of clinical acute care experience.
• Licensed Registered Nurse (RN) in the appropriate state.
• Comprehensive knowledge of Microsoft Office applications.
• Managed care experience.
• Passionate about improving consumer experiences.
• Experience with care management preferred.
• Auto dialer experience preferred.
