Overview

Form Health is a virtual obesity medicine clinic delivering multi-disciplinary evidence-based obesity treatment through telemedicine. We are hiring experienced Nurse Practitioners who are passionate about and who have recent experience in obesity medicine to provide longitudinal telehealth care for patients with obesity.

In Short

• Provide longitudinal telehealth care for patients with obesity.
• Collaborate with physicians and registered dietitians.
• Full-time remote position with some in-office presence.
• Requires Nurse Practitioner degree with experience in obesity medicine.
• Ideal candidates are licensed to practice in Tennessee.
• 2+ years of experience in outpatient or obesity medicine preferred.
• Excellent interpersonal skills are necessary.
• Telemedicine experience is a plus.
• Bilingual candidates in Spanish are preferred.

Requirements

• Nurse Practitioner degree.
• Experience in obesity medicine.
• Effective evidence-based lifestyle counseling skills.
• Ability to prescribe medication legally in Tennessee.
• 2+ years in outpatient or obesity medicine settings.
• Strong interpersonal skills.
• Telemedicine experience.
• Bilingual in Spanish preferred.
