Spanish Speaking LPN / LVN - Remote

Overview

We are seeking a dedicated and compassionate Spanish-speaking LPN/LVN to join our remote team as a contractor for the delivery of Remote Therapeutic Monitoring (RTM) and Chronic Care Management (CCM) Services, helping to improve patient outcomes through technology.

In Short

• Fully remote position with a schedule of 40 hours/week.
• Facilitate patient enrollment into RTM and CCM programs.
• Make daily calls and texts to document patient health status.
• Assist in collecting and documenting patient health information.
• Send health pointers to support patients.
• Maintain confidentiality and adhere to HIPAA regulations.
• Participate in ongoing training and check-ins.
• Requires LPN certification and 3+ years of experience.
• Proficiency with technology and strong organizational skills required.

• Certification as an LPN.
