Requirements

• 3 years of experience in healthcare coordination or community health.
• Experience in a team-based care approach.
• Excellent knowledge of community resources.
• Strong prioritization and task-switching abilities.
• Self-directed with problem-solving skills.
• Strong communication and teaching skills.
• Intermediate MS Office skills including Outlook and Teams.
• Able to work effectively in a remote environment.
• Must have a stable internet connection.
• Ability to work in a dedicated space to protect patient information.
