Requirements

• Master's degree in Social Work from an accredited institution.
• State licensure as a Licensed Clinical Social Worker (LCSW) required.
• 2-4 years of clinical experience, preferably in healthcare settings.
• Renal experience preferred.
• Behavioral health and/or hospice experience preferred.
• Bilingual preferred.
• Strong interpersonal and communication skills.
• Ability to provide empathetic support in a remote work setting.
• Familiarity with end-of-life care principles and community resources.
• Compassionate and self-directed attributes.
