Overview

As a DaVita Integrated Kidney Care Registered Nurse (RN) Case Manager, you will support complex patients in navigating the healthcare system, identifying their needs, and implementing appropriate interventions.

• Full-Time RN Case Manager position in Vacaville, CA region.
• Hybrid-Remote work model with telehealth and in-person visits.

Requirements

• Current RN License required; BSN preferred.
• Minimum of 2 years RN experience with healthcare performance coaching.
• Current CPR or BLS certification.
• Willingness to travel within an assigned territory.
• Home office with internet connectivity required.
• Preferred: 2+ years of Case Management experience.
• Spanish-speaking preferred in some markets.
