In Short

• Deliver Medical Nutrition Therapy (MNT) to patients with chronic conditions via video, phone, email, and portal.
• Conduct comprehensive nutrition assessments and develop individualized care plans.
• Use Medically Tailored Meals as educational tools.
• Leverage Remote Patient Monitoring data to optimize patient outcomes.
• Provide evidence-based lifestyle counseling and behavior change strategies.
• Manage an independent caseload with scheduled telehealth appointments.
• Monitor patient progress and maintain accurate documentation.
• Ensure compliance with HIPAA and regulatory requirements.
• Participate in mentorship sessions and access specialty training.
• Collaborate with peers through case consultations.

Requirements

• Bachelor’s degree in nutrition, dietetics, or a related field required.
• Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential required.
• Active state licensure in AL, FL, NC, NJ, NY, PA, or VA required.
• 2+ years of clinical nutrition experience required.
• Bilingual (Spanish and English) required.
• Experience with EMR systems required.
• Strong interpersonal and communication skills.
• Experience in chronic disease management preferred.
• Certification in obesity and weight management preferred.
• Patient-centered approach with cultural sensitivity.
