Overview

The Pharmacy Care Navigator collaborates with a managed care team to enhance medication adherence and connect members with pharmacy resources.

In Short

• Collaborate with interdisciplinary roles to minimize treatment gaps.
• Provide medication reconciliation and adherence services.
• Act as a liaison between members, care teams, and pharmacies.
• Assist with copay assistance program enrollment.
• Perform administrative and clinical support tasks.
• Educate care teams on medication access resources.
• Screen disease state registries for member eligibility.
• Schedule and follow up on pharmacist consult visits.
• Participate in quality initiatives to support clinical outcomes.
• Engage in medication safety projects and manage drug shortages.

Requirements

• Associates Degree and Pharmacy Technician Certificate.
• Minimum 6 years of pharmacy technician experience.
• Experience with EHR, G Suite, and medical terminology.
• Knowledge of ICD-10/CPT codes and third-party billing.
• Independent, strategic thinker eager to learn and grow.
• Medical clearance for immunizations required.
