Overview

The Therapist in the Collaborative Care Department provides outpatient community-based support and therapy to clients with mental health needs, working as part of a multidisciplinary team.

In Short

  • Provides individual diagnostic assessments and formulates treatment plans.
  • Delivers individual and group therapy, both in-person and via telehealth.
  • Offers crisis management and intervention for clients experiencing acute symptoms.
  • Collaborates with treatment team members and external providers.
  • Maintains professional licenses and documentation as per agency standards.
  • Engages in training opportunities and professional development.
  • Participates in consultation meetings and committees.
  • Provides Family Training and Counseling services as needed.
  • Adheres to the Continuum of Collaborative Care Model.
  • Eligible for incentive pay upon exceeding productivity expectations.

Requirements

  • Master’s degree in a mental health field required.
  • Licensure as LCMHC, LICSW, or LMFT preferred.
  • Experience with IDD and/or brain injury is preferred.
  • Proficient in Microsoft Office and electronic health records.
  • Ability to work independently and as part of a team.
  • Strong communication and organizational skills.
  • Ability to handle routine and emergent clinical situations.
  • Experience with co-occurring disorders is preferred.
  • Ability to document effectively in clinical work.
  • Willingness to learn and adapt to new clinical skills.
