Overview

This position assists clients in determining eligibility, completing program applications, and enrolling in programs to facilitate access to medical care, medication access, insurance, and housing, providing non-acute short-term supportive services for individuals living with HIV and their family members.

In Short

• Serve as a member of a collaborative multi-disciplinary client support team.
• Provide benefits counseling to assist clients in accessing medical pharmaceutical and insurance programs.
• Work with clients to gather necessary components for various medical insurance applications.
• Coordinate and track applications, referrals, and payment requests.
• Maintain client medical records and documentation as required.
• Meet with clients by phone/telehealth, in the office, and in the community.
• Participate in training to maintain up-to-date knowledge of assistance program eligibility.
• Represent Caracole in the community.
• Identify and pursue educational opportunities for professional growth.
• Other duties as assigned.

Requirements

• Associate’s degree with at least one year of experience in a social service or healthcare setting.
• Basic knowledge of HIV/AIDS and entitlement programs.
• High level of attention to detail and organizational skills.
• Basic computer skills and familiarity with Microsoft Office.
• Ability to maintain confidentiality in communications.
• Good verbal and written communication skills.
• Sensitivity to persons living with chronic disease.
• Bilingual (Spanish/English) preferred.
