Overview

The Patient Navigator supports the care of patients by assisting them in accessing healthcare services and achieving their health care goals.

In Short

  • Perform direct outreach to patients for engagement and enrollment.
  • Proactively contact patients to resolve barriers to appointments.
  • Leverage Motivational Interviewing techniques to engage patients.
  • Document patient communication in the Electronic Medical Record (EMR).
  • Verify and update patient insurance information.
  • Provide general clerical support including appointment scheduling.
  • Answer incoming calls and escalate patient issues as needed.
  • Contribute to workflow improvements and new ideas for the program.

Requirements

  • High School Degree or GED required; college study preferred.
  • No prior experience necessary; customer service experience preferred.
  • Strong communication and interpersonal skills required.
  • Ability to work in a team and independently.
  • Experience in healthcare or community-based settings preferred.
  • Completed motivational interviewing training preferred.
  • Ability to maintain confidentiality and cultural sensitivity.
  • Familiarity with Microsoft Office applications.
