As a Per Diem Health Coach at 9amHealth, you will be part of a team dedicated to improving the lives of individuals managing chronic conditions such as diabetes and obesity through personalized coaching and support.

• Provide comprehensive coaching for diabetes, hypertension, hyperlipidemia, and weight loss.
• Create consults aligned with internal protocols.

Requirements

• Active health coaching or related certification.
• Understanding of cardiometabolic conditions.
• 5 years of experience in direct patient care.
• Familiarity with EMRs and virtual health platforms.
• Strong communication skills.
• Experience in interdisciplinary care teams.
• Comfort delivering care virtually.
• Secure home workspace meeting privacy standards.
• Stable high-speed internet connection.
