Overview

The Case Management Analyst is responsible for developing, implementing, and promoting health service strategies that enhance the quality of healthcare delivery to members through local home and community visits.

In Short

  • Field-based position requiring local travel (75%) in Texas.
  • Conduct face-to-face visits and comprehensive assessments of members.
  • Develop and implement care plans in collaboration with healthcare teams.
  • Utilize clinical judgment and critical thinking for effective care management.
  • Coordinate and advocate for member services and resources.
  • Document care activities accurately in electronic health records.
  • Mentor new hires after achieving proficiency in the role.
  • Engage members in their healthcare decision-making process.
  • Ensure compliance with regulatory and accreditation guidelines.
  • Participate in interdisciplinary meetings for case reviews.

Requirements

  • 2+ years of experience in behavioral health or social services.
  • Bachelor's degree or non-licensed master's level clinician required.
  • Managed care experience preferred.
  • Experience in case management and discharge planning preferred.
  • Strong assessment and communication skills are essential.
  • Ability to use critical thinking and problem-solving.
  • Experience working with behavioral health members preferred.
  • Knowledge of healthcare systems and services.
  • Skills in motivational interviewing and member engagement.
  • Ability to work independently and as part of a team.
