Licensed Practical Nurse - Value Based Medicine - Remote

Overview

The VBM Licensed Practical Nurse will work in a telehealth capacity as part of a care team to improve the execution of quality requirements, increase throughput, improve efficiency, and enhance the overall patient experience.

In Short

• Full-time position, Monday to Friday 8 am to 5 pm.
• Option to work one day per week remote.
• Provide Medicare Annual Wellness visits to patients over the phone.
• Document all patient care/interactions in medical records.
• Track and log laboratory and diagnostic tests.
• Assist with patient inquiries for prescriptions and appointments.
• Participate in electronic documentation of care.
• Maintain BLS certification.
• Minimum of one-year professional nursing experience required.
• Perform other duties as assigned.

Requirements

• Certification as a Licensed Practical Nurse with current license in NC.
• Graduate of an accredited school of practical nursing.
• Previous medical office experience preferred.
• Maintain American Heart Association or American Red Cross BLS certification.
• Completion of Hepatitis B and Flu Vaccine forms required.
• Completion of TB test required.
