RN Transplant Nurse II (Weekends) - Remote

Overview

The RN Transplant Nurse II is responsible for providing case management and medical management for members receiving transplant services, ensuring quality healthcare and effective resource use.

In Short

• Fully remote position with a requirement to be within 50 miles of an eligible location.
• Day-shift hours including weekends.
• Conduct assessments and develop care management plans.
• Coordinate internal and external resources for patient needs.
• Monitor and evaluate care management effectiveness.
• Collaborate with healthcare providers and medical directors.
• Facilitate member care transitions through the healthcare continuum.
• Requires AS in nursing and 3 years of clinical experience.
• Preferred qualifications include transplant experience and case management certification.
• Must be willing to obtain licensure in multiple states.

Requirements

• AS in nursing and minimum of 3 years of clinical experience.
• Current unrestricted RN license in applicable states.
• Previous dedicated transplant experience is a must.
• Experience in Case Management and/or Medical Management is highly desired.
• Ability to interpret member contracts and managed care products preferred.
• Willingness to obtain compact nursing license.
• Certification as a Case Manager is preferred.
• Previous remote work experience is beneficial.
