Requirements

• Active Psychiatric Mental Health Nurse Practitioner (PMHNP) license in Vermont
• 2+ years prescribing experience
• Active DEA and individual NPI number
• Submission of a fully completed CAQH application
• Reliable internet connection
• Independent liability insurance with minimum coverage
