Licensed Clinical Social Worker - Remote

Overview

The Licensed Clinical Social Worker (LCSW) provides support to patients with complex health needs and is an integral part of our coordinated care team which includes physicians, nurse practitioners, nurses, care coordinators, social workers, dietitians, educators, and pharmacists. The Strive LCSW must be able to work successfully both in-person and remotely via telephone or videoconferencing technologies. The Strive LCSW is responsible for addressing non-clinical barriers to care and screening for social determinants of health. This position will report to the Sr. Manager, Clinical Care.

• Uses theories of human behavior and environment to conduct comprehensive psychosocial assessments and provides supportive counseling tailored to individual patient need.
• Establishes themselves as an authority on the social determinants of health and psychosocial aspects of kidney care.
• Locates and maintains resources for a wide range of community services.
• Uses motivational interviewing and behavior change techniques to develop patient-centered goals.
• Collaborates with multidisciplinary treatment teams to navigate across the healthcare ecosystem.
• Educates and supports patients and families through adjustment to chronic illness.
• Minimum qualifications include a Master’s degree in Social Work (MSW) and state-specific social work licensure.
• Preferred qualifications include strong organizational skills and the ability to develop relationships.
• Annual base salary range is $74,000 - $90,000.

Requirements

• Master’s degree in Social Work (MSW) required.
• 1+ years experience working in a clinical setting.
• State-specific social work licensure to practice clinically (LCSW, etc.).
• Proof of passing Association of Social Work Boards (ASWB) Exam.
• Efficient and reliable transportation, including an active driver’s license.
• Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down).
