Overview

The Remote Patient Monitor is responsible for analyzing and documenting remote patient data, monitoring vital signs, and communicating with healthcare providers to ensure timely intervention.

In Short

  • Monitor patients’ health and vital signs using RPM technology.
  • Alert healthcare providers of any concerning changes.
  • Act as a liaison between patients and healthcare providers.
  • Educate patients about the RPM program.
  • Screen patients for eligibility for services.
  • Conduct monthly clinical reviews via phone.
  • Ensure proper configuration and maintenance of monitoring devices.
  • Document patient information in electronic health records.

Requirements

  • Valid RN or LPN/LVN license and BLS certification.
  • Excellent oral and written communication skills.
  • Strong attention to detail and organizational skills.
  • Ability to manage multiple tasks without supervision.
  • Proficient computer literacy and typing skills.
  • Strong customer service skills.
  • Knowledge of medical terminology.
