Health Advisor - Remote

Overview

The Health Advisor at Privia Health will provide care coordination and support for at-risk patients, working closely with healthcare teams to improve patient outcomes.

In Short

• Conduct post-discharge follow-up calls with patients.
• Review and reconcile discharge medications.
• Assist in scheduling follow-up appointments.
• Identify high-risk patients for care management.
• Review Emergency Department discharges.
• Address Social Determinants of Care needs.
• Complete patient's open quality gaps.
• Demonstrate strong medical knowledge and critical thinking.
• Manage multiple tasks with attention to detail.
• Comply with HIPAA regulations.

Requirements

• Certified Emergency Medical Technician (EMT) or Medical Assistant (MA).
• 3-5 years of experience preferred.
• Experience in an outpatient healthcare setting preferred.
• Familiarity with electronic medical records (EMR) systems.
• Proficient in basic technology platforms.
