Join Our Team as a Nurse Care Manager! This remote opportunity allows you to support patients through every stage of their healthcare journey by coordinating care, ensuring timely follow-up, and promoting evidence-based practices.

In Short

• Remote opportunity for residents of Oregon, Washington, and Idaho.
• Compensation: $30-$37/hour, depending on experience.
• Full-time schedule: 4 days per week, 9-hour shifts; or 5 days per week: 4 8-hour shifts and one 4-hour shift.
• Serve as a patient advocate and navigator.
• Review clinical documentation for personalized patient education.
• Complete follow-up calls and offer education on alternative access points.
• Facilitate medication education and adherence support.
• Identify and support high-risk patients through proactive care coordination.
• Collaborate with interdisciplinary care teams.
• Maintain compliance with state licensure requirements.

Requirements

• LPN certificate/diploma, Associate Degree in Nursing (ADN), or Bachelor of Science in Nursing (BSN).
• At least three years of clinical experience in ambulatory care or care coordination.
• Proficient in EHR documentation.
• Experience with high-risk patient populations preferred.
• Current LPN or RN license in good standing in the state of practice.
