Intake Coordinator - Home Health - Remote

Overview

The Intake Coordinator is responsible for accepting incoming calls for new referrals for clinical services provided to home care patients within an assigned geographical area.

In Short

• Receives clinical information on new patient referrals to the home health agency.
• Collects all data pertinent to the request for admission including demographic data, medical history, medication, and insurance information.
• Verifies insurance/payer’s source via DDE or specific system.
• Ensures referral acceptance and communication within 15 minutes of receiving referral.
• Notifies clinical designee responsible for scheduling and authorizations.
• Liaisons between field, medical, office and referral source staff.
• Obtains authorization from insurance companies and ensures physician licensure compliance.
• Conducts follow up on current active patient hospitalizations.
• Maintains non-admit log.
• Attends in-services and meetings.

Requirements

• High school diploma or GED equivalent.
• Prior clinical home health experience preferred.
• Knowledge of Medicare, TennCare, and accreditation standards.
• Proficient in Microsoft Office Suite including Outlook, Word, and Excel.
• Familiarity with medical terminology.
• Excellent verbal, written, organizational, and time-management skills.
• Strong initiative and attention-to-detail.
• Demonstrated confidentiality and professionalism.
