• Support members through complex care management and disease management.
• Ensure members receive longitudinal care for excellent health outcomes.
• Work in a telephonic capacity.
• Report to the Manager, Care and Case Management.

Requirements

• Registered Nurse (RN) license required.
• Experience in care management preferred.
• Strong communication skills.
• Ability to work independently and as part of a team.
• Knowledge of healthcare systems and resources.
• Empathy and a passion for patient care.
• Ability to manage complex cases effectively.
• Proficient in using telecommunication tools.
• Strong organizational skills.
