Homeless Services Liaison - Remote

Overview

The Homeless Services Liaison plays a critical role in supporting Medicaid beneficiaries experiencing homelessness by connecting them with necessary resources and services.

In Short

• Collaborate with community organizations and healthcare providers.
• Conduct intake interviews and assessments of homeless beneficiaries.
• Advocate for homeless individuals within the Medicaid system.
• Collect and analyze data related to Medicaid services utilization.
• Develop individualized care plans in coordination with case managers.
• Maintain a housing resource guide for available support.
• Support outreach and engagement efforts with homeless individuals.
• Educate healthcare providers on challenges faced by homeless individuals.
• Travel across Michigan for face-to-face meetings with members.
• Participate in policy and program development related to homelessness.

Requirements

• Bachelor’s degree in social work, public health, or related field.
• Two years of professional experience in social services or community outreach.
• Experience with homeless populations and community resources.
• Strong decision-making skills and ability to work in crisis situations.
• Valid state driver's license and personal vehicle insurance required.
• Ability to travel within Michigan for member meetings.
• Knowledge of Medicaid policies related to homelessness.
• Preferred: Master’s degree in relevant field.
• Preferred: Bilingual or multilingual proficiency in specific languages.
• Must pass TB screening as part of the role.
