TeleHealth Nurse Practitioner - Remote

Overview

The TeleHealth Nurse provides telephone triage and professional nursing care to individuals or defined patient populations through the use of information and communication technologies and with the support of clinical guidelines, protocols, and other resources. Working from a home office, the TeleHealth Nurse uses critical thinking, therapeutic communication, and clinical decision-making skills to assess, plan, implement, educate and evaluate the patient’s health situation and/or informational needs.

In Short

• Documents care provided in the medical record accurately utilizing appropriate Home TeleHealth Clinics/Notes.
• Develops forms and questionnaires to facilitate the exchange of information.
• Sees that requested lab/tests are ordered, and results are sent to specialists.
• Assists patients with self-care teaching as required, and with maintaining as much independence as possible.
• Assesses, plans, implements, and evaluates the Home TeleHealth patient care delivery system.
• Schedules follow-up appointments.
• Provides psychosocial support to patients/family/caregivers through telephone care, direct patient care, or referral to appropriate support services.
• Utilizes assessment findings from all involved disciplines in formulating a plan of care.
• Evaluates the appropriateness of referrals and provides feedback to physicians.
• Establishes contact with patient/proxy and assesses willingness, ability to use Telemedicine.

Requirements

• Clinical nursing experience, with an active license and good standing as a Nurse Practitioner.
• Diploma from recognized and/or accredited nursing school. Bachelor degree is preferred.
• Excellent communication, telephone and customer relation skills.
• Good critical thinking, sound clinical decision-making and problem-solving skills.
• Basic knowledge and competence in the use of computers and keyboarding.
• Fluency in English (oral and written) is required and fluency in English and Spanish is preferred.
