Guidehealth is seeking a licensed Clinical Case Manager to enhance member management quality and assist in navigating the healthcare system.

In Short

• Work closely with clients and members to promote wellness.
• Conduct telephonic outreach and data analysis.
• Manage case assignments and documentation.
• Assist members in overcoming barriers to health.
• Educate members on healthcare navigation.
• Support operational aspects of the division.

Requirements

• Current IL LCSW, LCPC, or LPC.
• Minimum of five years in healthcare settings.
• Knowledge of case management principles.
• Excellent clinical judgment and communication skills.
• Strong organizational and problem-solving skills.
• Ability to work independently and remotely.
• Proficient in MS Office and EMR systems.
• High-speed internet and secure home office setup.
