Admissions Specialist - Remote

Overview

The Admissions Specialist is responsible for coordinating the smooth intake of all admissions and maintaining communication with various stakeholders including referral sources, case managers, and insurance companies.

In Short

• Full-time position with a schedule of 7:30 a.m. to 4:30 p.m., Monday to Friday.
• Coordinates patient intake processes and serves as a liaison between hospitals and facility programs.
• Notifies physicians regarding referrals and potential admissions.
• Manages ongoing communication with payers for continued stay approvals.
• Coordinates care plans among hospital staff, physicians, patients, and families.
• Participates in safety initiatives and completes safety-related competencies.
• Assures medical orders and necessary equipment are arranged prior to admissions.
• Responds to inquiries and identifies community resources as needed.
• Performs other duties as assigned by management.
• Five years of relevant experience required, preferably in a rehabilitation or acute care setting.

Requirements

• Associate’s degree required; Bachelor’s degree preferred.
• Experience in long-term acute care, sub-acute care, or hospice is preferred.
• Strong communication and coordination skills are essential.
• Ability to manage documentation and share information effectively.
