Overview

The Assessment Specialist role at CareStar involves working directly with individuals to assess their needs, develop personalized care plans, and connect them with essential services and supports in Lucas County, Ohio.

  • Minimum one-year experience with chronic conditions or three years with mental illness.
  • Knowledge of Medicaid and behavioral health systems.

Requirements

  • Bachelor's Degree in social work, counseling, psychology, or related field.
  • Registered Nurse (RN) or Licensed Social Worker (LSW) preferred.
  • Experience with chronic conditions or severe mental illness.
  • Effective interpersonal skills for diverse populations.
  • Strong knowledge of community resources.
