Overview

The Community Resource Navigator-Community Health Worker for the Charleston area Sea Islands Health Equity Project is responsible for outreach to patients, providers, and community-based organizations, screening for and coordinating social care for under-resourced patients from diverse backgrounds.

In Short

  • Responsible for outreach to patients and community organizations.
  • Screen and coordinate social care for under-resourced patients.
  • Collaborate with healthcare professionals to address socio-economic barriers.
  • Build relationships with patients and community organizations.
  • Conduct assessments and program enrollment for high-risk patients.
  • Input data into electronic medical records and other platforms.
  • Provide education about community resources.
  • Interact with patients in-person and remotely.
  • Work hybrid model: onsite, in community, and remotely.
  • Participate in data analysis efforts to improve health outcomes.

Requirements

  • Bachelor's degree in public/community health or related field.
  • Minimum of one year experience in a community-based setting.
  • Completion of Community Health Worker certification within one year.
  • Valid driver鈥檚 license and acceptable motor vehicle record.
  • Good organizational and time-management skills.
  • Ability to work independently and exercise sound judgment.
  • Available for occasional evenings and weekends.
  • Ability to travel to meet patients in various settings.
