Overview

The Community Habilitation Team Mentor provides support in a community setting to promote independence for individuals with intellectual and developmental disabilities, while assisting in the operations of the Community Habilitation Program.

In Short

Maintain a caseload of 15-20 hours of direct services.
Conduct intakes and meet & greets.
Coordinate Community Habilitation social groups and activities.
Complete staff observations and integrity checks.
Train new staff with their clients.
Oversee staff schedules and training.
Ensure compliance with OPWDD regulations.
Communicate regularly with managers and individuals served.
Meet monthly with the Program Director.
Perform other duties as assigned.

Requirements

2 years' experience as a Direct Support Professional.
1 year of supervisory experience.
Valid Driver's License and reliable transportation.
High School diploma.

Benefits

Full-time position with a pay rate of $20 per hour.
Potential for remote work after 90 days.
