Overview

VitalCheck Wellness is seeking a fulltime Licensed Practical Nurse (LPN) for our Diabetes and Chronic Disease Management Program, to coordinate the program, make sure there is patient adherence and provide education to employees of our clients in the NY and NJ area.

In Short

• Coordinate between patient, clinical and operations partners.
• Ensure high adherence rate to the program.
• Provide education around diabetes management.
• Support clients in diabetes management and well-being.
• Maintain clients documentation.

Requirements

• Bachelor’s degree in nursing or related field.
• Active LPN license in NY or NJ preferred.
• 3-5 years of experience in nursing or health administration.
• Experience with educating on chronic conditions.
• Proficient with computers and EMR systems.
• Bilingual in Haitian Creole or Spanish preferred.
